Working aloft Permit and Precautions

Content of Working aloft Permit

  1. Name of vessel
  2. Location: Sea\Port\Anchorage 
  3. Reason
  4. Date
  5. Permit Valid:    To and From 
  6. Any other permit (click here)  which will be used ( enclose space, cold work, Hot work , isolation permit  etc)
  7. Last Cargo
  8. Tank cleaning required 
  9. Concentration of Gases : Benzene\H2S\HC\O2\ Other gases as per MSDS
  10. Responsible officer: Name\ Rank
  11. Engine Room watchkeeper: Officer \ Crew
  12.  Deck Watchkeeper :Officer \ rank
  13. Present \ Predicted Weather : Wind \ Swell\ Current
  14. Signature of Responsible Officer
  15. Signature of Standby Crew
  16. Signature of Master

Precautions

  1. Tool box meeting should be conducted
  2. Risk Assessment should be done.
  3. Illumination should be adequate.
  4. Ventilation should be running.
  5. Proper PPE should be worn (please refer PPE blog).
  6. Safety Officer should take rounds.
  7. If working aloft has to be done in Enclose space then please refer Enclose space blog.
  8. If Hot work has to be done when working aloft then please refer Hot work blog.
  9. Tools should be lifted up or lowered by using Bucket.
  10. Standby Person should not stand vertically done where work is going on.
  11. FFA equipment (Portable extinguisher, Fire blanket, Fire Hose and Pressurized Fire line) should be standby.
  12.  If working on Main Mast/ Funnel/ Fwd Mast below equipment should be isolated                                                                                                1) Whistle  2) Radar Scanner 3) Navigational Light 4) Antenna etc (Isolation Permit should be made) 
  13. Notice board should be placed near equipment on bridge, E\rm  etc regarding "MEN AT WORK"
  14. Lock out and Tag out procedure should be used. 
  15. Safety equipment is kept standby. 
  16. One person should be standby at all times.
  17. Communication between Bridge\Standby person \ECR should be checked 
  18. Walkie\Talkie channel decided for communication.
  19. Reporting interval should be decided.
  20. Permit has to be filed for records.
  21. Entries in Log Book should be made.

While maintenance near Radar Scanner(in addition to Above Points).

  1. Manufacture repair checklist should be used.
  2. Isolation permit should be issued.
  3. If Magnetron of Radar is to changed and is a critical spare (see blog), Requisition of spares should be made and sent to office.  

While Painting (In addition to Above Points).

  1. Adequate personnel should be available.
  2. Wooden staging should be satisfactory condition and safely rigged.
  3. Wooden staging should be of correct size.
  4.  Ropes used for staging should be of satisfactory condition.
  5. Paint drum and brushes should safely secured.
  6.   Wooden staging should not be painted.
  7. Wooden staging should be used where there is proper support.
  8. Wooden staging should be used in parallel body length of ship and not at Ford and Aft Part (Curve Part). 
  9. If painting has to be done overside for hull or draft painting then 1) Overside permit has to be issued 2) Rudder\Propeller should be isolated 3) Overboard valve should be isolated 4)Anchor should be secured 5) If in Port, Port permission will be required 6) etc.  
  10. When painting funnel, ship should be stopped and  temperature of funnel should be lowered.
  11. Correct  Paint for High Temperature should be applied at Funnel.
