Enclose Space entry Procedure and Precautions

On a ship there are various Enclose space (Click Here).

Enclose spaces are those spaces

1. Where There is limited openings for entry and exit
2. Which has Inadequate ventilation
3.  which is not designed for continuous worker occupancy

Content of Enclose space entry Permit

1. Name of vessel
2. Location: Sea\Port\Anchorage 
3. Reason
4. Date
5. Permit Valid:    To and From ( Maximum period of validity should not exceed 12 hours)
6. Any other permit (click here)  which will be used ( Hot work, working aloft, squeezing etc)
7. Last Cargo
8. Tank cleaning required 
9. Concentration of Gases : Benzene\H2S\HC\O2\ Other gases as per MSDS
10. Responsible officer: Name\ Rank
11. Engine Room watchkeeper: Officer \ Crew
12.  Deck Watchkeeper :Officer \ rank
13. Present \ Predicted Weather : Wind \ Swell\ Current
14. Signature of Responsible Officer
15. Signature of Standby Crew
16. Signature of Master

Precautions

1. Tool box meeting should be conducted
2. Risk Assessment should be done.
3. Illumination should be adequate 
4. Ventilation should be running 
5. Gas meter  (click here) should be calibrated before use.
6. Gas check should be done at three levels (Top\Mid\Bottom)
7. Gas check should be done 10 minutes after Ventilation is stopped. 
8. Safety equipment is kept standy   1.EEBD (Emergency escape breathing device)    2.SCBA (Self contained Breathing device)    3. Neil Robortson stretcher   4. Tripod     5. First aid Kit
9. Communication between Bridge\Standby person \ Crew entering into enclosed Spaces is checked 
10. All Line and valve of associated enclosed spaces should be isolated.
11. Walkie\Talkie channel decided for communication.
12. Personnel Gas meter should be properly calibrated
13. Reporting interval should be decided.
14. A crew should be standby at the entrance of Enclose space.
15. Crew entering Enclose space should be briefed about the Hazards present in Enclose space.
16. Crew member entering the space should sign the enclose space permit
17. If ventilation is stopped, all crew member should exit the enclose space
18. Permit become Void if ventilation stops.
19. Once work is over and all crew is out of the space,  Permit has to be closed
20. Below entry should be recorded in Enclose entry log (click here) such as                                                                                                                    1.Time of entry      2. Time of exit      3. Gas concentration     4. Torches\ Illumination         5. Gas meter
